All guests must have a completed and approved guest permission form.  Forms are available outside of D107.

• You must have this form completed and turned into Mrs. Merli by March 15, 2022. 
• No late forms will be accepted.
• Guests must provide photo identification at the door.
• No student who attends middle school or individuals 21 or older will be allowed admittance.
• Please print neatly except for requested signatures.
• Don't wait until the last minute! The form must be signed by the guest's school administrator or employer.
