Pre-Participation Sports Forms
HCS Middle and High School students planning on participating in interscholastic athletics during the 2013-14 school year must have a pre-participation physical on file in their high school's athletic office/athletic training room before they can participate in practices and games. Physicals for the 2013-14 school year are valid from April 1, 2013 - June 30, 2014. Students and parents are encouraged to obtain a physical as soon as possible in anticipation of the upcoming sports seasons. Forms are available through your school's athletic training room or may be accessed here.
NEW Online Physical Forms!
If you are a parent of a student-athlete or your child is thinking about participating in a sport at their middle school or high school, please take note of this change.
Horry County Schools is transitioning to an electronic version of the pre-participation physical packet. All forms that have been completed by hand and signed by a parent and student will now be completed online.
We are conducting a trial to make sure that this new process is going to work. Please assist us by completing these forms and submitting them for review.
The physical portion must still be completed and signed by a physician, physician’s assistant, or nurse practitioner and returned to your child’s coach or athletic trainer.
In the future, this process will be completed at the time you register your child for school. Please click here then complete and submit the forms.
The link below will take you to the old form.
If you should have any questions or concerns about this process, please do not hesitate to contact the Athletic Trainer at your child’s high school.
Physical forms MUST be signed by a licensed medical doctor in South Carolina or a Certified Physician’s Assistant or Family Nurse Practitioner practicing under the supervision of a licensed South Carolina MD or DO. Chiropractor signatures are NOT valid!!
Also, ALL forms must be completed in full, signed, and dated where indicated. Incomplete forms will NOT be accepted and students will not be able to participate until all completed forms are on file.
Turn ALL forms to the sports medicine department at your child’s high school before their first practice.
