FMLA - Application for Family Medical Leave
To initiate the FMLA process, please complete the FMLA Application Form and submit it to your principal for signature. After obtaining the signature, the form will be sent to Human Resources Department unless you prefer to submit it directly. This form helps us understand the reason for your absence, the expected duration of your leave, and other relevant details.
