Immunization Requirements

South Carolina law requires that every student enrolled in school have a valid South Carolina Certificate of Immunization or an approved exemption on file. Students who do not have the required documentation will not be permitted to attend school, and absences resulting from noncompliance with this requirement will be considered unlawful.

The Statewide Immunization Online Network (SIMON) is South Carolina's secure, confidential electronic immunization registry maintained by the South Carolina Department of Public Health (SCDPH). SIMON contains immunization records entered by authorized healthcare providers, local health departments, other immunization providers, and South Carolina schools. Authorized providers can issue a valid South Carolina Certificate of Immunization for students who meet state immunization requirements. Parents or guardians may obtain this certificate from their child's healthcare provider or local health department.

  • All students enrolled in kindergarten (5K) through grade 12 in South Carolina public and private schools must meet the immunization requirements for their grade level. Current immunization requirements are available on the South Carolina Department of Public Health (SCDPH) website:

  • South Carolina recognizes only two types of immunization exemptions:
    • Medical Exemption – May be issued only by a licensed South Carolina healthcare provider when medically appropriate.
    • Religious Exemption – May be obtained through a local South Carolina Department of Public Health (SCDPH) health department, either in person or through the online exemption process.

  • Parents and guardians are responsible for ensuring their child meets South Carolina immunization requirements and for providing the required documentation to the school. If your child needs immunizations to meet school requirements, schedule an appointment with your healthcare provider or your local SCDPH health department as early as possible.
